Overview of Portal Forms (Surveys)

ManagementStudio portal forms, known as Surveys, are used to collect information within Projects. Data gathered through these forms is stored in project-specific custom forms and fields. For example, you can create a form to record desk layout preferences, such as monitor configuration, telephony equipment, or other requirements. Surveys can be configured to collect user input for various project modules, including:

  • Applications
  • User Migrations
  • Devices
  • Mailboxes
  • Bespoke
  • Deployment Units
  • Defects
  • Tasks
  • Contacts

Survey configuration consists of five main areas, each detailed below.

Form Setup

The Form Setup section contains the basic settings for portal form surveys. Key configuration options include:

  • Button labels
  • On-screen messages
  • Custom triggers
  • Survey type (e.g., Questionnaire, Pass/Fail test)
  • PDF attachment generation for completed forms

You can define welcome, completion, and instruction messages for the survey. Surveys can also be configured to generate PDF copies of responses for audit purposes.

Form Builder

Use the Form Builder to define the questions and fields included in the survey. Surveys are designed to collect data relevant to specific deliverables within each module. Information can be stored in:

  • General Fields
  • Custom Form Fields

For example:

  • In the Applications module, survey application owners about licensing or usage.
  • For Users, collect migration requirement details.
  • For Devices, gather feedback after migration.

Email Options

Survey processes can include automated emails, which are configured in this section. Available email templates:

  • Request Email: Introduces the survey and provides a link to access it
  • Reminder Email: Sent if the survey is incomplete after a specified period
  • Complete (Confirmation) Email: Sent upon survey completion, detailing next steps or summary

Emails support custom HTML, Markdown formatting, and variable substitution. You can configure reminders to be sent after a defined number of days.
